AGREEMENT FOR ATTENDANCE AT CONFERENCES

As you register for this event, it is important that we are clear from the start about what will be expected. These policies, procedures and practices are designed to help you to get the most out of the experience. 

ETHICS You can count on The Institute for Physician Wellness (IPW,) its consultants, and staff to uphold the highest standards of ethics and professionalism.

COPYRIGHTED MATERIALS The event materials are the property of The Institute for Physician Wellness. You are asked to agree that you will not use or reuse materials for any financial gain or otherwise distribute to any third parties without the express written permission of The Institute for Physician Wellness. Copyrighted material included from outside parties is outside the scope of this limitation.

PHOTO RELEASE By enrolling in the event, you are granting IPW permission to use your image and/or voice in videos, photographs or recordings in publications, videos, mass media, educational events, research, service, or public relations activities for IPW.  In addition, you are agreeing to release and waive IPW and its employees and consultants from any liability for use of media and waive any remuneration.

REFUND POLICY Cancellations made 90 days prior to the event will be refunded the full registration amount minus a nonrefundable $250 processing fee. All cancellations made for any reason after that date are nonrefundable. No exceptions or transfers can be made. The Institute for Physician Wellness, LLC reserves the right to cancel or postpone any event due to unforeseen circumstances. In the unlikely event that Kathy Stepien is unable to be on-site for the retreat (family emergency, acute illness, and similar), every attempt will be made to on-board a suitable substitute before rescheduling or canceling the event. In the unlikely event of a cancellation or postponement, The Institute for Physician Wellness, LLC will refund the registration fee minus the non refundable $250 processing fee, but is not responsible for any other related costs or charges to the participant. The host facility is a separate entity from The Institute for Physician Wellness and has a separate cancellation policy.  The Institute for Physician Wellness is not responsible for fees or charges associated with your cancellation at the facility. Waiver of Liability 

I voluntarily agree to participate in activities with The Institute for Physician Wellness, LLC. I hereby assume all risks of injury to me and my property that may be sustained in connection with the event and hold harmless The Institute for Physician Wellness, LLC, its owner, consultants and employees.  I understand there is an element of risk in any educational, coaching, and conference activity. I assume full responsibility for myself for any injury, death, complication, loss of personal property and expenses thereof.  I understand that Dr. Stepien is not expected to nor able to provide medical or psychological care. I agree that if I require medical care, any costs incurred are my sole responsibility and not that of The Institute for Physician Wellness, LLC.  I have read this agreement and fully understand its contents. I agree to it with my own free will. I am of legal age and accept the above disclaimer and authorization.
